“To reduce the number of lower extremity amputations and improve the quality of life of our fellow citizens who are afflicted with wounds and complications from diabetes and peripheral arterial disease.”

We will accomplish these goals by using advanced evidence-based methods, by educating health care practitioners and patients, through community outreach, philanthropy and patient advocacy.

You may donate via our website, as well as our Facebook site. Additionally, checks may be mailed to our mailing address: 108 Melrose Court, Ponte Vedra Beach, Florida 32082. Lastly, if you shop on Amazon, please designate us as your charity of choice on Amazon Smile. Each time you shop on Amazon, go to Smile. Amazon donates .05% of every dollar you spend directly to our philanthropic patient assistance account.

Not at this time, but we have plans to create an online store with a variety of items and gear that will show your support of SALSAL!

Contact us. Any donation would be greatly appreciated and will go directly into our philanthropic patient assistance account.

At present, there are no Chapters or plans to create local Chapters of the Foundation. We are a network of like-minded healthcare providers and private citizens who recognize the importance of our mission and the magnitude and scope of the problems we are working to improve.

Most common requests have been for diabetic shoes and protective insoles, Roll-a-bout walkers (knee scooters), socks and wound supplies. We will consider all requests.

Typically, those in need of assistance are identified by health care providers during patient encounters in wound centers or private offices. Providers may contact us or print off a copy of our Patient Assistance voucher, complete it and return to us by e-mail or postal service. Our Board of Directors reviews requests for assistance and reimburses the vendor providing the requested service.

Yes. We are a 501c3 non-profit organization.

All private donations go directly into The Save A Leg, Save A Life Foundation’s philanthropic account. Funds are used as part of our patient assistance program, where those in need receive help in obtaining services and resources that further our mission.
