NOTE: This program is currently on hold pending funding. The Save A Leg, Save A Life Foundation is committed to helping those in need with items such as diabetic shoes, dressing supplies, knee walkers, or other items used to help wound healing and prevent lower extremity amputation.

The Patient Assistance Program is funded by our donors and sponsors, through monetary and in-kind donations. Patients in need are typically referred by a wound care center or other health care provider. Assistance could include:
- Dressings
- Diabetic shoes
- Socks and compression socks
- Walkers, canes, and roll-about walkers
- Ankle braces
- Inserts, orthotics, and prosthetics
This program does not provide direct cash assistance.
The Patient Assistance Program requires an application including financial, insurance and medical information. This information will be kept confidential. To be considered for assistance, please download and return the application.
